The general **Billing Information Form** is provided here as an Microsoft Excel spreadsheet. This should be downloaded, filled out and [emailed to us](/contact_owner) when submitting a sample or seeking other services. For users who do not have Harvard's 33-digit internal billing code, outside rates will be applied. In addition, if our on-line paperless sample submission system is not being used, a sample submission form should be filled out and[ emailed](/contact_owner) to us.
