The general Billing Information Form is provided here as an Microsoft Excel spreadsheet.  This should be downloaded, filled out and emailed to us when submitting a sample or seeking other services.  For users who do not have Harvard's 33-digit internal billing code, outside rates will be applied.  In addition, if our on-line paperless sample submission system is not being used, a sample submission form should be filled out and emailed to us.
